problem when using the field header title in add form

Hi
I created a form and I tried to put the fields in 2 columns, things worked very well, but when I wanted to group the fields by title, the form became disorganized and it's set the field in a single column instead of 2 colomns;
i am using phprad classic 2.7.3 and window 7 xampp .

could you help please?
