input radio button and upload file, can't work in two or more field

i'm create a form with two field each field using input radio button, but when  i'm traying to input, onliy one field can eccept the selection , the other field don't work
it happen again when create 2 or more field, each one using upload file, only work in the first filed, the other filed don't work
any suggestion for this problem?
